Job Summary:
Under general supervision, manages the effective dispensing, processing, and recording of prescriptions given by doctors, dentists, and other approved health care providers. Provides customer-focused pharmaceutical care. communicates with service providers to help shape the best possible drug information for patients and medical professionals. engages in patient and provider education initiatives to influence patient compliance and provider prescribing habits for the better. oversees and gives direction to the team of pharmacy technicians as they carry out technical tasks. Assists in cash and inventory control in line with departmental policies, keeping all records as required by law for the practice of pharmacy.
Essential Responsibilities:

analyzes the requirements for patient care and services in a way that makes it easier to realize organizational care and service initiatives as well as a high degree of customer satisfaction.

Reviews of prescribed drugs and dosages guarantee that treatment is secure and suitable for the patient's condition. identifies clinically significant medication interactions and allergies using patient profiles. Prescriptions are processed, labeled, and given the correct identification information and usage instructions. correctly mixes and delivers medication.

gives patients guidance and consultation regarding adverse drug reactions, precise storage needs, and thorough usage instructions. oversees the compliance and responsiveness of chosen patients to medication in order to enhance patient outcomes.

regularly interacts with healthcare professionals to encourage the use of formulary drugs in accordance with clinical guidelines, target drug programs, and pre-approved therapeutic substitutes. records all clinical interventions using forms that have been approved. takes part in all actions related to the clinical endeavor, such as testing or necessary monitoring.

shows competence using computerized systems for entering medication orders, reviewing patient profiles, or retrieving clinical or statistical data. keeps accurate records of prescription files, controlled substance records, non-formulary records, medication profiles, and other legal and departmental data. demonstrates competence with cash management procedures and the Point Of Service (POS) cash register system.

acts as a point of contact for the allocated clinical care team, demonstrating problem-solving abilities with regard to medication-related difficulties, input on drug education issues, and encouraging interdepartmental cooperation on pharmaceutical care programs.

Attends and participates in clinical team meetings, continuing education programs that are accredited by ACPE, and other activities that maintain professional competency. aids in the teaching and training of pharmacy technicians and students.

Contributes to the team or departmental projects aimed at achieving enhanced service effectiveness and patient outcomes, and takes part in departmental quality improvement activities.

participates in pharmacy vaccination procedures, such as administering shots and documenting them in members' permanent health records.

does other tasks as directed.

Basic Qualifications:
Experience
  • Minimum one (1) year of experience as a pharmacist in an outpatient clinic, hospital or retail pharmacy.
  • Per the National Agreement, current KP Coalition employees have this experience requirement waived.
Education
  • Bachelor's Degree in Pharmacy or Pharm.D. from an accredited college of pharmacy.
License, Certification, Registration
  • Basic Life Support
  • Pharmacy-Based Immunization Delivery Certification
  • Pharmacist License (Maryland) within 6 months of hire
  • Anaphylaxis and Epinephrine Auto-Injector Certification
  • Pharmacist License (District of Columbia) within 6 months of hire
  • Pharmacist License (Virginia)

 

Additional Requirements:

  • being able to carry out all of a pharmacist's duties and interpret and comprehend prescriptions.
    excellent communication skills both in writing and speaking.
  • Must be able to function in a partnership between labor and management.
  • Credentials from several states are required for this position.
  • Before being hired, credentials from the major employment state are required. After hiring, additional credentials from the secondary work state(s) are needed.
Preferred Qualifications:
  • Home IV experience preferred – IV experience highly preferred.
  • Completion of an ASHP-accredited pharmacy residency program is preferred.
