Job Summary:
In accordance with the nursing code of ethics, the ambulatory nursing care standards, and the regulations of the Kaiser Foundation Health Plan of Washington, provide nursing care to a specified set of patients. Organize and facilitate the patient's access to healthcare services in other areas of the healthcare delivery system. Together with the healthcare team, provide and organize nursing care for the patient. The Registered Nurse oversees and participates in patient assessment, care plan development, and implementation as a crucial part of the health care team.
Essential Duties:
- A defined set of patient's nursing care needs are continuously assessed using the nursing process. This examination is described using a nursing diagnosis. creates a written plan of treatment to meet the needs of the patient. helps patients, their families, and other important people to create healthcare goals that are a component of their unique treatment plan. based on the most important medical demands of the patient and the available funding. delivers nursing interventions in a safe and knowledgeable manner. Prior to interventions, information is given to the patient's family/significant others. provides patients' family members and other important people with telephone consultations. continuously assesses how well patients are responding to treatment. recognizes and prepares for changes in the patient's condition and nursing care needs. Modifies the individual plan of care in collaboration with the health care team.
- By examining, planning, implementing, and evaluating care to fulfill the requirements of the populations served, one demonstrates comprehension of the physiological and psychological aspects of normal aging. Effective communication abilities with the targeted age groups serviced are displayed. identifies one's own learning needs and engages in educational initiatives according to the patient groups serviced according to their age (Neonatal, Pediatric, Adolescent, Geriatric). collaborates, coordinates, and interacts with other members of the healthcare team to plan and deliver care. evaluates the needs of specific patients who need hospitalization; starts nursing admission database; collaborates with inpatient staff and doctors on release plans. Effectively collaborates and interacts with other team members to complete the job.
- In every nurse-patient interaction, the nurse conveys that they are aware of the patient's psychological, cultural, and emotional requirements. Introduces patients, their families, and other important people to the role of the professional nurse and other members of the healthcare team, the nursing services offered, and the methods for accessing those services. orients people to their obligations under the Consumer Bill of Rights. as part of the nursing assessment, determine the patient's educational needs. conceives of an educational element for each nursing care plan. teaches and counsels patients in accordance with their educational needs and those of their families, close relationships, and the medical treatment plan. helps patients follow their treatment plans and prescriptions. Science and behavioral knowledge are the foundation of education.
- Examines risk factors related to patient lifestyle. combines the constant assessment of nursing care needs with components of care that promote health and act as preventative measures. develops a strategy for improving the overall health of the patient. adopts preventive measures that are both psychological and physical. encourages patients to take care of themselves and be accountable for their own medical needs. complies with KFHPW documentation requirements. complies with KFHPW safety standards and offers a secure environment for each patient. follows the KFHPW standards and recommendations for infection control, including those for reporting communicable diseases.
- Recognizes personal learning requirements and engages in programs for ongoing education to address such needs. takes part in student instruction and staff orientation events. collaborates with managers to complete the performance assessment process for oneself, teammates, and teams. By creating, putting into practice, and assessing professional nursing standards as well as taking part in multidisciplinary quality assurance activities, nurses ensure the quality of care. engages in professional pursuits. takes part in nursing research and applies what is learned to nursing practice. respects KFHPW personnel policies and conducts themselves in a highly professional manner at all times. consistently exhibits an appearance that adheres to the KFHPW dress code.
Basic Qualifications:
- N/A
- Associate's degree in Nursing from an accredited program.
- Registered Nurse License (Washington) required at hire
- Basic Life Support required at hire
- Communication, problem-solving, leadership, and decision-making skills.
- Ambulatory care nursing experience
- Primary nursing or case management experience.
- Bachelor of Science in Nursing (BSN).
