Description:
The Psychosocial Clinician II is a member of an interdisciplinary team that offers members and/or family members with direct evaluation, diagnostic formulation, treatment planning, intervention, and psycho-education. Direct patient care will take up most of the job's time.
Significant Duties:
Provides members with a rapid and accurate diagnosis, treatment planning, and therapeutic intervention.
Adult, teen, and/or kid patients are treated.
Gives members who are presenting problems the proper therapy option.
Demonstrates proficiency with a range of therapy approaches.
Provides excellent internal and external customer/patient satisfaction and effectively communicates healthcare material to members and clinical professionals.
Gives members' charts accurate and timely documentation of clinical visits.
Conduct a preliminary interview with the members while gathering and analyzing the psychological and social context of the complaints.
Determines the members' needs' urgency.
Gives a qualified analysis of the member's presentation, suggests a therapeutic course of action, and chooses the best treatment option.
Gives members who are in need of professional services or who are experiencing a crisis the right guidance and therapy at the right time.
Maintains a written record of therapy, writes reports and letters, calls social services, schools, family, and friends (with member permission), and visits them to get essential information.
Provide clinical advice on family interactions, sociocultural influences, and community resources in a setting of integrated health care.
Provides resource information, counseling, and referrals over the phone.
Knowledgeable about local resources and KP.
Adept in recognizing and managing the range of developmental stages.
Adept at using a variety of therapeutic methods, including but not restricted to an individual, group, family, and child therapy.
Complies with all regulatory agency policies and procedures as well as organizational, departmental, state, and federal norms.
Follows all statutory requirements for patients' clinical care and licensing.
Keeps a license.
Engages in CQI and QM operations.

EOL (End of Life Option):

- Regulatory data reporting, process coordination, assist Members/Physicians with the California End of Life Option Act.
- Two (2) years of post-graduate experience in a clinical setting are required.
- Medical Social Worker – End-of-Life Option: If the position posted is for an End of Life role, the experience requirement is: Minimum of one (1) year of experience, within the last three (3) years, working as an MSW in end-of-life care and counseling.
- A Master's degree in Social Work, Psychology, Sociology or another field related to behavioral health is required.
- Licensed Marriage and Family Therapist (California) OR Licensed Clinical Social Worker (California)
- National Provider Identifier
- May consider LMFT, with a Master's Degree in a Behavioral Health field, job code 66564 for this position. Following language applies to Home Health, Hospice, and Palliative position ONLY: Current CA driver's License, clear driving record for the past two (2) years, and proof of automobile insurance as required by law.
- AHA BLS card required.
- LCSW required.
- Knowledge of and ability to utilize current DSM nomenclature required.
- Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to customers, contracted providers, and vendors.
- Must be familiar with all aspects of psychopathology and recognize the appropriate and timely clinical treatment intervention needed.
- Must be familiar with and have expertise in a variety of treatment modalities.
