Description:
Manages patient drug therapy, provides patient education and prepares and/or oversees the preparation and administration of medications under indirect supervision. Provides ambulatory pharmaceutical care services utilizing authorized protocols in coordination with other health professionals. encourages the provision of cost-effective, high-quality pharmaceutical treatment. carries out these tasks in the designated clinical areas.

Significant Duties:

Promotes logical and affordable inpatient and outpatient drug therapy under the guidance of the department manager or designee through close collaboration with assigned physicians and other medical staff during the drug therapy decision-making process.

gives guidance to pharmacy technicians, pharmacy residents, pharmacy interns, and other non-licensed staff.

on the recommendation of doctors and other medical professionals, inquires into the drug history of patients.

Create a treatment plan based on the diagnosis, which could involve pharmacological therapy and lifestyle changes, and move between the various sites along the continuum of care.

identifies, addresses, and prevents real and potential medication-related issues, such as untreated indications, poor drug choice, subtherapeutic dosage, overdose, failure to take medicine, unfavorable drug reactions, drug interactions, and medication taken without a prescription.

Based on data gathered from patient interviews, objective monitoring metrics, reviews of medication profiles, and examinations of the patient's medical record, follows protocol for starting, adjusting, monitoring, and recommending changes to drug therapy.

Organizes and interprets laboratory testing for drug therapy, such as electrolytes, serum drug levels, and other authorized drug-related tests.

offers patient monitoring services in order to assess and improve patient outcomes, drug use and compliance, identify and/or reduce ADRs, and stop wasteful and redundant drug use.

Educates patients on their disease state, including expected behaviors, side effects, justifications for appropriate compliance, and any particular instructions.

Assure that the given pharmaceutical care services continue to perform better.

presents writes newsletters, conducts formal utilization evaluations, and has one-on-one conversations with workers to educate them.

carries out additional tasks as needed.

Basic Qualifications: 
Experience
  • Successful completion of a clinical pharmacy residency or possessing the equivalent clinical experience in direct patient care delivery.
Education
  • B.S. degree in Pharmacy OR Pharm.D required.
License, Certification, Registration
  • Pharmacist License (California) required at hire
  • Basic Life Support from American Heart Association

 

Additional Requirements:
  • ACLS certification may be required in some areas.
  • Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
  • Working knowledge of aseptic technique and preparation of sterile IV products and chemotherapy agents. Knowledge of emergency medications and drug monitoring.
  • Understanding of drug utilization review and drug usage evaluation
  • Knowledge of Federal and State laws & regulations regarding the provision of pharmaceutical services.
  • Possess excellent verbal and written communication skills
  • Must be able to operate the pharmacy computer system(s) and other equipment. Must be able to input prescription labels.
  • Must obtain an NPI # within one month in position.
  • Must be able to work in a Labor/Management Partnership environment.
