Job Summary:
In order to help patients and their families cope with the social, emotional, and financial challenges linked to medical conditions or which predispose to sickness, the Medical Social Worker II assesses, coordinates, and implements/evaluates professional social services. The duty for treating more complicated and challenging cases, including but not limited to crisis intervention and behavior modification, sets this classification apart from the Medical Social Worker I classification.

Significant Duties:

upholds the policies, procedures, and guiding principles of Kaiser Permanente as well as any relevant local, state, and federal laws.

Clinical Service Quality

In accordance with departmental and medical center policies, as well as the I standards of the social work profession, the Medical Social Worker II develops and implements treatment programs after analyzing the needs of the patient/family system.

QM results, case presentations, documentation reviews, staff and patient comments, and supervisory discussions all serve as indicators of the quality of clinical services.

according to departmental policies and regulations, conducts a bio-psychosocial evaluation interview with the patient, their family, and/or their significant other.

In urgent situations, provides adults, kids, and families with appropriate crisis intervention and treatment, including assessment, counseling, information/referral, and consultation to doctors and the medical staff.

explains the patient's problem in a professional manner (i.e., the circumstance, attitude, behavior, and verbal content) and makes recommendations for the best psycho-social intervention(s) and/or treatment strategy.

Creates a culturally appropriate assessment that adheres to departmental standards and includes the following information: the informant's name, the source of the referral, the reasons for the referral, physical and medical issues, interpersonal and social relationships, emotional and psychiatric issues, including assessments of depression, role performance and social transition, community planning and resources (as indicated), and legal and protective issues (as indicated)

based on clinical evaluation and congruent with patients' ongoing medical needs, provides the right counseling services to patients and/or their families.

does essential patient and family advocacy work for KP and the neighborhood.
Individual, group, or classroom patient education is given on topics relevant to psycho-social coping with medical disease.

The ability to effectively apply clinical knowledge of bio-psychosocial development stages, personality development, human behavior, etc. to formulate assessments, guide treatment, and offer consultation and education to other team members exemplifies the integration of clinical social work theory into practice.

Basic Qualifications:
Experience
  • N/A
Education
  • Masters Degree in Social Work.
License, Certification, Registration
  • Registered Associate Clinical Social Worker (California) OR Licensed Clinical Social Worker (California)
Additional Requirements:
  • Following language applies to Home Health, Hospice and Palliative positions only.
  • Current CA Drivers License, clear driving record for the past 2 years and proof of automobile insurance as required by law.
  • One year post masters experience in Medical Social Work required.
  • BLS card required
Preferred Qualifications:
  • One-year experience in a health care environment preferred.
