Significant Duties:
A defined set of patient's nursing care needs are continuously assessed using the nursing process. This examination is described using a nursing diagnosis. creates a written plan of treatment to meet the needs of the patient. helps patients, their families, and other important people to create healthcare goals that are a component of their unique treatment plan. based on the most important medical demands of the patient and the available funding. delivers nursing interventions in a safe and knowledgeable manner. informs the patient's other family members of earlier interventions. provides patients and their loved ones with telephone consultations and conducts continuing assessments of how well patients are responding to treatment. recognizes and prepares for changes in the patient's condition and nursing care needs. Modifies the individual plan of care in collaboration with the health care team.
By examining, planning, implementing, and evaluating care to fulfill the requirements of the populations served, one demonstrates comprehension of the physiological and psychological aspects of normal aging. Effective communication abilities with the targeted age groups serviced are displayed. identifies one's own learning needs and engages in educational initiatives according to the patient groups serviced according to their age (Neonatal, Pediatric, Adolescent, Geriatric). collaborates, coordinates, and interacts with other members of the healthcare team to plan and deliver care. evaluates the needs of specific patients who need hospitalization; starts nursing admission database; collaborates with inpatient staff and doctors on release plans. Effectively collaborates and interacts with other team members to complete the job.
By creating, putting into practice, and assessing professional nursing standards as well as taking part in multidisciplinary quality assurance activities, nurses ensure the quality of care. engages in professional pursuits. takes part in nursing research and applies what is learned to nursing practice. respects KFHPW personnel policies and conducts themselves in a highly professional manner at all times. consistently exhibits an appearance that adheres to the KFHPW dress code.

- Associate's degree in Nursing from an accredited program.
- Registered Nurse License (Washington) required at hire
- Basic Life Support required at hire
- Communication, problem-solving, leadership and decision-making skills.
- Ambulatory care nursing experience
- Primary nursing or case management experience.
- Bachelor of science in Nursing (BSN).
